Report Code of Conduct Issue

Purpose
We are committed to maintaining a safe, respectful, and inclusive environment for everyone. This form allows you to report any behavior that you believe may violate our Code of Conduct, including harassment, discrimination, bullying, retaliation, or other unprofessional conduct. Your report helps us investigate and address concerns promptly and fairly.

Confidentiality
All information you provide will be treated with the utmost discretion. Reports are shared only with the designated trust and safety team or compliance officers responsible for handling investigations. You may choose to remain anonymous; however, providing your contact information can help us follow up for clarification or to offer you support.

Reported by
Reports should include details of the incident, date, time, location, and any witnesses. Upon receipt of a report, the Conduct Committee will acknowledge receipt within 48 hours. The Committee will conduct a fair, timely, and impartial investigation, including interviews with involved parties and witnesses. The individual(s) under investigation will be given the opportunity to respond to allegations.

Once submitted, you will receive an acknowledgment within two business days. Our team will review your report and may reach out to you for further information. We strive to complete investigations in a fair, timely manner and will keep you informed of progress, subject to confidentiality constraints.
